We are a non-profit organization with a mission to work in all areas of the social determinants of health to identify community barriers, spread awareness, and create sustainable change for rural communities. We service a five-county catchment area, including Marshall, Putnam, Woodford, Stark, and Henry. Our vision is to cultivate growth and enhance the quality of rural communities for generations to come.
